Oxygen stores, preoxygenation and the oxyhaemoglobin dissociation curve
This is a Primary physiology viva. You are about to anaesthetise a healthy 30-year-old man weighing 70 kg for an elective laparoscopic appendicectomy. Your consultant asks why you preoxygenate every patient and how long he could safely be apnoeic. You will be shown an oxyhaemoglobin dissociation curve. Answer the examiner's questions, relating the physiology to anaesthetic practice. Use numbers where you can.
- Describe the body's oxygen stores breathing air and after preoxygenation.
- Explain the factors that determine the time to desaturation during apnoea, and the mechanism of apnoeic oxygenation.
- Describe the oxyhaemoglobin dissociation curve, its shifts and their clinical relevance.

Write the answer. Get it marked.
15 short-answer questions · 150 minutes · each marked out of 10 from 2026.2. Every answer is marked point by point against a rubric that rewards what ANZCA examiners reward: specific points, the exact question answered, safe advice.

FEx SAQ · marked against the rubricAn 84-year-old woman with severe aortic stenosis (valve area 0.7 cm², mean gradient 48 mmHg) needs a hemiarthroplasty for a fractured neck of femur. Outline your plan for induction of general anaesthesia. (4 marks)
“A gentle, cardiostable induction with careful titration of drugs. I would maintain her blood pressure with metaraminol and avoid hypotension and tachycardia. Post-op HDU.”
- ✗Invasive arterial monitoring established before induction0/1
- ✗Named agents with doses reduced for age and fixed output (e.g. propofol 0.5–1 mg/kg titrated, fentanyl, rocuronium)0/1
- ✓Vasopressor (metaraminol or phenylephrine) prepared or running before induction1/1
- ✗Stated haemodynamic target, e.g. mean arterial pressure within 20% of baseline, heart rate 60–80, sinus rhythm preserved0/1
Examiner note: "Gentle" and "cardiostable" score nothing. The marker wants the arterial line before induction, the drugs and doses you would give this patient, and the target you are defending. Examiners flag these phrases in every Final report.
