4:00 reading, then 7:00 in the station, as on the day. Patients, relatives and examiners are played by an AI so you can practise alone, or a colleague runs the station with the script, the findings and the marking sheet.

Syncope in a young man: risk-stratifying history
You are the emergency registrar in a metropolitan ED. Mr Josh Tran, 26, has been brought in by ambulance after collapsing while playing indoor soccer. He was unconscious for "less than a minute" according to the paramedic handover and is now feeling well. His observations are normal, his 12-lead ECG is being repeated and will be handed to you by the examiner if you ask for it, and his capillary glucose was 6.1 mmol/L. He wants to go home to work a night shift.
- Take a focused history from Mr Tran to assess the cause of his collapse and his risk.
- Ask the examiner for any results you need.
- Tell Mr Tran your assessment and your plan in the final minute.

Short-answer paper · 3 hours · computer-based · reported as up to 30 questions. Every answer is marked point by point against a rubric that rewards what ACEM examiners reward: specific points, the exact question answered, safe advice.

SAQ · marked against the rubricA 24-year-old woman presents 9 hours after an intentional ingestion of 40 tablets of paracetamol 500 mg (20 g); she weighs 60 kg. List your immediate management of the paracetamol ingestion. (4 marks)
“This is a significant overdose. I would give supportive care, check a paracetamol level and liver function tests, start N-acetylcysteine if the level is above the nomogram line, and refer to psychiatry.”
- ✗Start acetylcysteine now, without waiting for the level: presentation is more than 8 hours after a potentially toxic ingestion (≥ 10 g or ≥ 200 mg/kg)0/1
- ✗Regimen stated: 200 mg/kg over 4 hours, then 100 mg/kg over 16 hours (two-bag)0/1
- ✓Paracetamol concentration and alanine aminotransferase (ALT) on arrival1/1
- ✗Plan for the level: if above double the nomogram line, double the second bag to 200 mg/kg over 16 hours; no activated charcoal at 9 hours0/1
Examiner note: The level does not decide whether to start acetylcysteine after 8 hours: the dose ingested does. "Start NAC if above the line" scores nothing here, and "supportive care" never scores. Name the regimen and the decision point.
