Exertional chest tightness and a request for a coronary calcium score
You are a GP in a suburban practice. Andrew Kelly, 54, an accountant, booked a standard appointment and has asked the receptionist to "add a heart calcium scan referral". His notes show he is an ex-smoker (quit 5 years ago, 25 pack-years), has treated hypertension (telmisartan 40 mg) and his total cholesterol 18 months ago was 6.2 mmol/L. He has not been seen for a year.
The examiner will discuss this case with you as if you were seeing him today. Treat the examiner as a colleague: explain your reasoning and what you would do and say.
- Discuss how you would assess Mr Kelly's symptoms and his request.
- Outline your investigation and interim management plan, including safety-netting.
- Discuss cardiovascular risk assessment for his asymptomatic brother.

Write the answer. Get it marked.
70 scenarios · one "choose 3" multiple-selection question each · 3 h 30 + 30 min allowance. Every answer is marked point by point against a rubric that rewards what RACGP examiners reward: specific points, the exact question answered, safe advice.

KFP · marked against the rubricGraham, 58, has had 40 minutes of central chest tightness at rest. He used sildenafil last night. Pulse 92, blood pressure 128/80. Choose the 3 most appropriate immediate actions in your clinic. (3 marks)
“Selected: aspirin 300 mg chewed; sublingual glyceryl trinitrate 300–600 microgram; 12-lead ECG.”
- ✓Aspirin 300 mg chewed (no allergy or active bleeding in the stem)1/1
- ✓12-lead ECG within 10 minutes of arrival1/1
- ✗Call an ambulance (000) for transfer to the emergency department: glyceryl trinitrate chosen instead, within 24 hours of sildenafil0/1
Examiner note: The stem told you he took sildenafil last night. Nitrates within 24 hours of sildenafil (48 hours of tadalafil) risk profound hypotension, so glyceryl trinitrate is wrong here, and it cost you the ambulance. Examiners report ignored stem details as the commonest KFP fault.
