Primary postpartum haemorrhage after vaginal birth: leading the response
You are the obstetric registrar on call overnight in a metropolitan Queensland hospital with an on-site blood bank and interventional radiology available by day only. You are called to birth suite room 4. Jasmine Taylor, 34, para 3, gave birth vaginally 15 minutes ago after an induced labour (oxytocin infusion for 8 hours). The baby weighed 4,150 g. The placenta delivered 5 minutes ago and appeared complete. The midwife estimates blood loss of 900 mL and says bleeding is continuing. The examiner will act as the midwife and give you information as you ask for it.
- Describe your immediate assessment and management of Jasmine to the examiner.
- Respond to the evolving clinical information you are given.
- Outline your actions after the bleeding is controlled.

SAQ · marked against the rubricTwenty minutes after a vaginal birth, a 34-year-old para 2 has lost 1,000 mL and her uterus is boggy. Her blood pressure is 100/60 mmHg. Outline your immediate management. (6 marks)
“Call for help. Give Syntocinon and put up IV fluids. Check bloods. Massage the uterus. If still bleeding she may need to go to theatre…”
- ✓Calls for help and escalates (senior obstetrician, anaesthetist, midwife in charge); two large-bore cannulae; bloods including crossmatch, full blood count, coagulation and fibrinogen1/1
- ✓Uterine massage or bimanual compression; empties the bladder1/1
- ✗Uterotonics named with dose, route and order: oxytocin 5 IU slow IV then infusion; ergometrine 250 micrograms if not hypertensive; carboprost 250 micrograms IM, repeated every 15 minutes up to 8 doses0/2
- ✗Tranexamic acid 1 g IV over 10 minutes, within 3 hours of birth0/1
- ✗Structure: resuscitate, identify the cause (tone, trauma, tissue, thrombin), treat, escalate to theatre, with a massive transfusion protocol trigger0/1
Examiner note: The content is not wrong, but it is an unstructured list with no doses. RANZCOG SAQ markers credit structure as well as content, and "give Syntocinon" earns nothing where the key asks for the agent, dose, route and sequence.
